STYPENDIUM STUDENCKIE LISTA KONIECZNYCH DOKUMENT�W	STUDENTS GRANT APPLICATION CHECKLIST
Do podania należy załączyć	Enclose the following information with your application
Załączyć dow�d rejestracji na rok akademicki - student jest przynajmniej na drugim roku studi�w	Proof of registration for academic year - you must be at least in your 2nd year of studies
Załączyć wyniki w nauce z roku poprzedniego	Grades from previous academic year
Załączyć opinię prezesa Okręgu KPK lub kuratora Funduszu Wieczystego Milenium względnie proboszcza miejscowej parafii	Include references from branch president CPC, or trustee or parish priest where there is no CPC organization
Kopię dochodu (Notice of Assessment of Income Tax) z roku poprzedniego osoby starającej się o stypendium oraz os�b wymienionych w podaniu w 3-ciej sekcji.	Copies of income and Notice of Assessment of Income Tax for previous year of the applicant and other persons (parents/guardians/spouse - where applicable) as shown in Section III in this application.
Podania kt�re nie będą całkowicie wypełnione i/lub bez koniecznych dokument�w nie będą rozpatrywane.	Incomplete applications and applications without the required supporting documents will be rejected.
Podania kt�re wpłyną po 15 września nie będą rozpatrywane.	Please note that the cut off date for applications is September 15th.  Applications received at the Canadian Polish Millennium Fund after the above date are deemed ineligible for consideration of student grants.
Wszelka korespondencja kierowana będzie na adres domowy. W przypadku zmiany adresu prosimy o poinformowanie Funduszu.	All correspondence will be addressed to the applicant's home address.  In the event of an address change, please inform the Foundation.

CRITERIA FOR CONSIDERATION

• Must be enrolled in second year or later at a recognized Canadian university or college
• Canadian students studying abroad may also be considered, depending on circumstances
• Proof of gross family income indicating financial need
• Copy of transcript from previous year of study
• Confirmation of enrolment by the university or college for continued academic studies
• Endorsement of the application by one of the following: a member of the Canadian Polish Millennium Fund; parish priest, the president of a branch of the Canadian Polish Congress

Applicants are encouraged to provide additional information related to their circumstances which may assist the Board of Directors in approving the application.

Applicants are encouraged to provide supporting documentation to demonstrate participation as a volunteer in a Canadian Polish parish, organization or institution.

Applicants who do not satisfy all of the above-listed criteria may provide reasons why they should still be considered.

Applicants who have an ancestor whom was a member of the Armia Krajowa/Polish Home Army (AK) should provide related information (history) as they may qualify for an additional bursary.

The awarding of bursaries is strictly at the discretion of the Board of the Directors.
